Ensuring safety for both the individual in crisis and those around them. - Provision of Quality Intervention in the Least Restrictive Setting:
Offering effective support while maintaining the most comfortable and least restrictive environment possible. - Development of an Immediate Safety Plan:
Creating a plan to help avoid the need for higher levels of care.
Community Stabilization
Goal of Community Stabilization Services
The goal of Community Stabilization services is to stabilize the individual within their community and support the individual and/or their support system during the following periods:
- Between Initial Mobile Crisis Response and Follow-Up Services:
Supporting the individual during the transition from an initial mobile crisis response to established follow-up services at the appropriate level of care. - Transitional Step-Down from Higher Level of Care:
Assisting as a step-down when a higher level of care service has been identified but is not immediately available for access. - Diversion to Prevent Higher Level of Care:
Offering support as a diversion to prevent the need for a higher level of care.
23-Hour Crisis Stabilization
23-Hour Crisis Stabilization
The 23-Hour Crisis Stabilization service provides up to 23 hours of community-based support for individuals experiencing a behavioral health crisis. This service focuses on assessment and stabilization interventions to ensure the individual receives the appropriate care.
- Accessible 24/7:
Available at any time to respond to crisis situations. - Indications for Service:
Ideal for situations where an individual is in acute crisis and requires a safe environment for observation and assessment before determining if admission to an inpatient or residential crisis stabilization unit is necessary.
